GST REBATE CLAIM FORM

INFORMATION SHEET

In order to claim GST you must have:

1. original receipt which identifies GST paid.

2. the vendor's GST number is clearly identified on the sales receipt.

GST may NOT be claimed for purchases made at any Girl Guides of Canada Guide

Shop.

Fill out the attached GST Rebate Claim Form and submit it with the original receipts

attached to the Guide Centre.

Cheques will be issued after receipt of funds from Revenue Canada.
